TAKE A GRADUATING STUDENT EXIT SURVEY
NOTE:

If you are completing a CERTIFICATE program ONLY, you are NOT required to fill out the survey.

If you are graduating from TWO OR MORE PROGRAMS, you only need to fill out the survey ONCE.
- You may need your Mason email ID and Mason email password to log into the survey.

You may download and print the survey and mail the completed form to the address listed on the form.
You may request a paper copy of the survey by emailing assessment@gmu.edu or
by calling (703) 993 8834.
To send you the correct version we need to know the following:
1. Name
2. Address
3. Senior/Graduate/Law Student
4 .Semester you expect to graduate.
